A granite monument, with a bas-relief of John J. McGraw in his Giant uniform, is surmounted by an oversized baseball inscribed with an interlocking NL for National League.

Text on the statue reads, "In memory of John J. McGraw born in Truxton April 7, 1873 died Feb. 25, 1934 a great American one of baseball's immortals dynamic leader of the New York Giants for 30 years."

Text on the back notes that, "on August 8, 1938 the New York Giants played an exhibition game with the Truxton ball team in this village which made possible the erection of this memorial"
